Overview
Documents in the Patient Profile help you keep important files attached to the patient record.
When you're working on a specific Patient, you can easily view and manage any documents associated with that Patient right from within the person's profile.
Patient Documents Queue
Navigate to a Patient's profile. In the banner, you'll see a tab for Documents.
This Document queue is specific for this Patient.
Note that this Patient Document queue is different from the Document Management queue (found in the upper right quick-links banner) which will populate all Documents in the system, beyond just one specific Patient.
Document Information
In the Patient Document queue, you'll see important information about each Document.
Document Type
Organizes Documents into Types to help categorize and organize patient-related files within the system. Document Types help standardize how Documents are labeled, making it easier to store, search, and manage records. Visit our help article on Document Types to learn more about creating custom types.

This section shows the name of the Document and file type.
Updated
This section shows the timestamp of the most recent update to the Document.
Document Queue Filters
You can use filters to easily find specific Documents.
In this example, you can filter Documents to surface anything uploaded in a specific time range. For more information on using filters in NikoHealth, visit our help article on Creating Custom Views with Saved Filters.
Document History
By default, the Patient Document queue will show you all active Documents for this Patient. But you can view the Patient's Document history to see anything that's been deleted by clicking the History Button.
While you're in the Document History, you'll see that the button changes to Active. Click Active to navigate back to all current Documents.
Taking Action on Documents in Patient Profile
From the Patient Document queue, you can take many actions on the Documents.
Click on the 3 dots at the end of each Document line for a menu of options. From here, you can edit/modify, view, download, share, or delete the Document.
You can also hover over any Document for similar options.
If you want to perform an action on multiple Documents, you can multi-select the Documents you need, and an action bar will appear at the bottom.
You can take bulk action to download, share, or delete multiple Documents.
Add New Documents to Patient Profile
To add a new Document to the Patient from within the Patient Profile, use the blue NikoHealth action button.
Select Add Documents. A new page will open where you can upload a Document or attach a Form.
Upload Document to Patient Profile
To upload a new Document to the Patient Profile, click or drag and drop the file. After it's been uploaded to the system, a preview will appear with some additional options.
Add Form to Patient Profile
Forms are Documents you can generate from NikoHealth for your Patients. To learn more, visit our help section on Forms.
When adding a Form to a Patient, you must select the Form template. Form templates are created in your Application Settings, and they allow you to pull specific data fields from NikoHealth to include in a Form.
