Overview
You can edit any form template in NikoHealth. Editing a template allows you to change the template name, description, and what fields in NikoHealth are mapped to.
If you want new fillable fields in the form, you will have to create a new form with the desired fillable fields.
Locate Form Templates
To access your forms, click Management in the top menu.
Select Application Settings in the left menu and choose Form.
A list of all forms will appear on the Form Templates page. Along the top are search fields you can use to locate the desired form.
You must use at least three characters to run a search and results are not case sensitive.
Edit a Form Template
Hover over the template you want to edit and click the pencil icon to the right.
The Edit Form window will open. You can now edit the Name, Description, and Field Mapping for the form.
