Generate a Form with Signature Options

Overview

If a form template was mapped with a signature field, you can collect a patient’s signature with a touchscreen, mouse, or plug-and-pay signature pad. For more information on creating form templates and mapping fields see the article Create a Form Template.

In this article, we’ll go over how to generate a form that can collect a digital signature and how to save the signature.

Remember that forms are patient-specific, to generate them, you must first navigate to a patient profile. Once in a patient profile, you’ll need to click the Forms tab.

Generate a Form with Signature Options

First, click the Template dropdown to find the form template you want to use. You can start typing the template name to narrow the dropdown options.

Once you’ve selected the form, you have the option of picking an order to associate with the form. Use the Order dropdown to link an order to the form.

Click the Sign Date field and select the calendar date for the form. If there is a referring physician's signature on the form, this is the date it will show as signed by them.

When you’re done, click Generate and Sign.

A PDF will open of the form template with applicable information pulled from the selected order. Other information will be pulled from the patient profile based on how you mapped fields.

You can click on any of the fillable fields and manually add additional information.

To get a signature, scroll to the area you mapped for a signature and click the field.

The screen that opens allows someone to create a digital signature. Digital signatures can be done using a mouse on a desktop or a touch screen on a mobile device. To clear the signature and try again, click Clear in the bottom left.

To save the signature and apply it to the form, click Accept Signature in the bottom right.

The signature will populate the field.

To back out of the signature screen without saving, hit the Esc key.

When you’re done click Save at the bottom of the PDF.

A window will open with an option to download the signed form.

The form will now be saved as a Document in the patient profile. For more information on patient documents, see the article Understanding Documents.
