Introduction

Forms are documents you can generate from the system for your patients. Forms can range from a patient consent form to a pickup slip for ordered items. You can generate internal forms for patients to complete, insurance authorization and appeal forms, and forms that referring providers may need to complete all in one place!

Forms will pull relevant patient information on demand and even allow for digital signatures. Take a look below at some of the key features of forms.

Using Forms

Forms allow you to pull key information from a patient profile while capturing other dynamic data. Below we’ll go over some scenarios for which forms can be used.

Form Scenarios

Patient Consent Form

You can create a formal patient agreement and consent contract and then create a fillable space for patients to digitally sign and date in the office. Instead of printing out each consent form, you can hand over a tablet ready to go where the form will be saved right to the patient profile.

Certificates of Necessity

Certain items may require a special form filled out for a patient's need and signed by a referring physician. By uploading the forms as customizable templates, you can pull the basic patient information and order information into generated forms.
