Once you’ve generated a form, you can view it at any time in a patient’s profile. Remember that forms are patient-specific, to view them, you must first navigate to a patient profile. Once in a patient profile, you’ll need to click the Forms tab.

In this article, we’ll go over how to look at previously generated forms.

You can see a list of all form templates under the Patient Profile Forms tab. To see what the form will look like for that patient, click the eye icon to the right.

The form will open as a PDF in a new tab. If the new tab doesn’t open, click Open in new window in the file-generated popup.

From here you can fill in any blank fields and Download or Print the form.
